Pilot for strukturert tverrfaglig oppfølgingsteam – en kvalitativ studie av kommunale lederes erfaringer med implementeringen

I 2018 lanserte Helsedirektoratet pilotprosjektet Strukturert tverrfaglig oppfølgingsteam med mål om å implementere nasjonal veileder for oppfølging av personer med store og sammensatte behov (2017) i seks kommuner. Veilederen retter seg mot ledere på alle nivå og tjenesteområder i kommunen som yter tjenester til personer med store og sammensatte behov, uavhengig av alder og diagnose. I utgangspunktet var piloten planlagt å vare ut 2021, men ble forlenget med to år til 2023. Fire kommuner valgte å fortsette prosjektet.

Artikkelen undersøker lederinvolvering i implementeringen av piloten basert på intervju med 44 topp- og mellomledere og utvalgte nøkkelpersoner i de fire pilotkommunene våren 2022, og drøfter dette i lys av implementerings- og ledelsesteori. Forskningsspørsmålene omhandler 1) hvor godt piloten har vært forankret blant ledere på ulike nivå og på tvers av sektorer, 2) hvilke ledergrep som er tatt, 3) hvilke utfordringer lederne har erfart, og 4) hvilke gevinster lederne ser.

Resultatene viser at innføring av oppfølgingsteam som arbeidsform har stor oppslutning blant toppledere i pilotkommunene. Piloten går inn på kjerneoppgavene til kommunene og understøtter utviklingsarbeid de uansett må gjøre. En rekke ledergrep er tatt på ulike nivå. Vi fant merkbart mindre involvering og kjennskap til piloten blant enhetslederne fra oppvekst og velferd enn blant enhetsledere i helse og omsorg. Lederinvolveringen utfordres av kompleksiteten i implementeringen og av at tjenesteområdene styres av ulike formål og rammebetingelser. Studien avdekker et behov for å rydde i mylderet av koordineringsmodeller og -begreper.

Abstract in English

In 2018, the Directorate of Health launched the pilot project Structured interdisciplinary follow-up team with the aim of implementing national guidelines for following up people with complex needs (2017) in six municipalities. The guidelines are aimed at managers at all levels and sectors in the municipalities providing services to people with complex needs, regardless of age and diagnosis. Initially, the pilot was planned to last until 2021, but was extended for two years until 2023. Four municipalities chose to continue the project.

The article examines managerial involvement in the implementation of the pilot based on interviews with 44 top and middle managers and selected key persons in four pilot municipalities in spring 2022, and discusses this in light of implementation and management theory. The research questions concern 1) how well the pilot has been anchored among managers at different levels and across sectors, 2) management measures that have been taken, 3) challenges the managers have experienced, and 4) benefits the managers see from the project.

The results show that the introduction of follow-up teams as a work methodology is supported by top managers in the pilot municipalities. The pilot goes into core tasks of the municipalities and supports the service development they need to do anyway. A number of management measures have been taken at various levels. We found noticeably less involvement with and knowledge about the pilot among managers from education and welfare services than among managers in health and care services. Managerial involvement is challenged by the complexity of the project and by the fact that the sectors are governed by different purposes and framework conditions. The study reveals a need to clear the myriad of coordination models and concepts.
